Question: Is there something in REAPER you regret having implemented the way you did because later on you wanted to change it but it's difficult/impossible for not breaking things etc.?

Tons of things, sure. Sometimes they get overhauled (the way regions were stored originally was as a pair of markers, ugh), other times not (MIDI items/editors/etc).
